摘要 | Emerging technology can significantly extend the preservation duration of cadaveric hearts for distant delivery. This new technology can potentially alleviate heart shortages by pooling donations across expanded regions and reducing waste, which requires additional financial investments and service network designs. In this paper, we propose an integrated model to jointly support strategic decisions (infrastructure and technology investment) and operational decisions (matching hearts and patients with different priorities) with guaranteed service levels (waiting time and treatment rate). Computationally tractable reformulations via Mixed-Integer Second-Order Conic Programs (MISOCPs) are proposed. With the data obtained from the United Network for Organ Sharing (UNOS), we conduct case studies to identify conditions under which the new technology investment can be levered up to improve the conflicting objectives between treatment rate and aggregate cost simultaneously. |
